We have recently launched our In-Home Therapy service line: a program specifically designed to provide the necessary therapy treatment to patients either after their Home Health discharge or those without a Home Health episode. Oftentimes, patients are still in need of ongoing therapy treatment after discharge, which can help to avoid rehospitalizations and ensure the patient is safely acclimated back to their home environment. In a worst-case scenario situation, should a patient demonstrate a need for additional care, we can refer them back to the skilled nursing facility or home health agency they were discharged from. In-Home Therapy ensures a patient is cared for beyond their in-patient stay and that their functional progress continues.
